"Round face" is one of the most common things people say in a first consultation, and it is rarely about weight. It is usually about structure: a chin that sits back, a jawline without a clear edge, cheekbones that do not catch the light.
Filler placed with restraint can change that without changing who you are.
This post follows one plan from start to finish. Jennifer (her name is changed for privacy, and she consented to sharing her story and photos) is a 22-year-old medical assistant who came to our Larchmont office wanting balance.

Why "balancing" and not "filler"
Filler is the material. Balancing is the plan. When I map a face, I look at how the chin, jawline, cheeks and lips relate to each other, because adding to one area changes how the rest reads.
A stronger chin, for example, can make lips look more in proportion before any lip filler is placed.
That is why Jennifer's plan started with the chin alone in October 2025, then returned to the cheeks, jawline, chin and lips together in September 2026.
Why Radiesse for the chin, jawline and cheeks
Radiesse is a calcium hydroxylapatite filler. It is firmer than hyaluronic acid gels, which suits areas that need support rather than softness. It also encourages your own collagen over the following months, so the structure it creates keeps building after the visit.
For Jennifer, that meant 1.5 cc to the chin in session one, then 0.7 cc to the cheeks, 0.8 cc to each side of the jawline and 0.7 cc more to the chin in session two.
The lips received 1 cc of Evolysse Smooth, a hyaluronic acid filler, because lips need movement rather than structure.
What it felt like from her side
Jennifer had mild swelling for about a day and went back to work the next morning. Her result was visible the same day, and she rated the experience 5 out of 5.
Her words in the room: "Omg!!! I look so good. I love my results. This is just what I wanted."
Radiesse results are commonly expected to last 12 to 24 months. Individual results vary, and the amounts above were chosen for her anatomy, not as a template.
